What Floors can you access the Secret ones from?  And why are there two Floor 18s?

When you grab the spear in Floor 18a it turns into Floor 18b immediately (notice how the middle area is the same).  I can't remember the floors that lead to the secret levels but they are two of the maps with three elevators.

It turns out there were two "level packs" - "Return to Danger" and "Ultimate Challenge" - for Spear of Destiny, each as big as the original game...

Hey, I just noticed that the Spear of Destiny maps all have the same typo in the credits as Wolfenstein 3D did...
